docs(create-headless-agent): sync with openrouter-web changes

Summary

Adds openrouter:shell to the server-tool checklist (default OFF) and to the wired-tools list in the README. It now appears in the public Responses and Anthropic Messages tool unions, so it is reachable through the serverTool(...) path this skill scaffolds.

Source: OpenRouterTeam/openrouter-web#31954, plus the ShellServerTool_OpenRouter entry in projects/docs/openapi/openapi.yaml.

The added note captures the non-obvious scoping: sandbox execution via openrouter:bash with engine: 'openrouter' is Messages-only, so openrouter:shell is what to use on Responses. Chat Completions carries neither.

Same change applied to create-agent-tui in a separate PR.
