Precompute immediate and recursive Git revision directory entries in one per-directory index.\n\nOn current ocaml/main, the 100k-file immediate lookup falls from 5.587 ms / 600k minor words to about 17 ns / 2 words; index construction rises from 94.57 ms to 113.07 ms.\n\nFollows the recursive-entry indexing from ocaml#11254.
