概要
issue-implementで issue コメントの確認を必須化し、本文との矛盾や未解決事項がある場合は Ready でも着手を止めるよう明記しました。worktree-startの issue 入力経路でもコメントを確認し、未解決事項がある Ready issue はissue-implementに自動連鎖しないよう整理しました。issue-pickの deep-read でコメントを確認し、要確認 issue を推奨 / 補欠から分離する出力形式を追加しました。検証
