This revises the BBR-Copilot integration for upstream LSQUIC:

exposes BBR-Copilot as a full congestion-controller option
supports engine-wide and per-connection selection
keeps Adaptive limited to Cubic and BBR
reuses Adaptive congestion-controller state when switching
documents configuration and the BBR-Copilot paper
adds focused send-controller lifecycle coverage

Verification: the full build succeeds and all 80 tests pass.

Intended to update litespeedtech#662 after merge.
