[FEATURE] Add OpenTelemetry distributed tracing

Implementation:

• Instrument all Express routes with OpenTelemetry spans.
• Propagate trace to indexer and SQLite operations.
• Export to Jaeger via OTLP.
• Trace IDs in log lines.

Acceptance Criteria:

• Full trace from HTTP to DB visible.
• Jaeger UI shows traces in Docker Compose.
• P99 traces captured.
