Update from code changes: vendo_apps_open response kinds

Summary

Documents what vendo_apps_open now answers over the MCP door: an open-in-product link for built apps, and structured pending/failed states for the build window.

Changes

• Added an "Opening a saved app" section to the door guide with a kind table (vendo/open-in-product@1, pending, failed), a code example branching on structuredContent, and the fallback text line.
• Noted that a sealed bundle links out to the deployment's public base URL (never its content hash), and that with no public URL configured the answer says the app is built and ready to open in the product.
• Clarified that an app awaiting build approval or still being built answers { kind: "pending", say } rather than an error, and a terminal build failure carries a readable reason plus a retryable hint.
